Several local hospitals, health departments, and community organizations are conducting a survey to learn more about how to improve life in your community. Tell us what you think most impacts the health of those that live in your community by taking an online survey with a trained community health worker. 

 $15 Stop and Shop gift card for those who participate. Limited to 25 surveys per location/day.
